Before you book your dog for daycare or boarding you must enroll your dog in our facility. Please take the following steps, which can be also done during your evaluation session. During the evaluation, we will give you a full tour of our facility from top to bottom.
You can also take this opportunity to ask as many questions as you want about our facility and our way of doing things. Don't be shy to make any special requests for we will try to accommodate you as best we can.
Step 1:Fill out the enrolment form and sign the waiver agreement (click here to download)
Step 2: Bring in copies of your dog's vaccination records or have your vet fax your dog's vaccination records prior to your evaluation. Fax# 718-499-5695
The following vaccinations must be UP TO DATE:
• Rabies
• Bordetella (Administered within the last 6 months as per NYS Law)
• Distemper
• Parvo
• Hepatitis
• Parainfluenza
Step three: Evaluation of the dog. To ensure that your dog will enjoy his stay at dogs den you must bring your dog in during the following times:
• Monday Thru Friday 11:30am-4:00pm 7:00pm-8:00pm
• Saturday & Sunday: 11:30am-4:00pm 7:00pm-8:00pm
We're constantly adding detail to your dog's personal profile, from noting his habits and diet, to working with your regular vet to keep an up-to-date medical file ready for emergencies – everything as you would. Our database system allows us to keep track of your vaccination records and we will notify you as your vaccinations expire.
It is important to note that it is your responsibility to keep the vaccination records up to date to ensure your dog's safety and everyone else's. You can request your vet to send us updates after each visit. We will turn you away if your vaccination records are not up to date.
*Please note that New York State Law requires Bordetella be updated every 6 months
